I am the parent or legal guardian of the athlete named below. I give permission for my child to participate in the Big Stage Analytics program, which uses STATSports Sonra wearable tracking technology and the Big Stage data platform.
I understand and consent to the following:
Collection of performance, load, location, heart rate, and recovery data during practices, games, and related activities
Use of this data by coaches, athletic staff, and Big Stage Analytics for performance improvement, injury risk reduction, and recovery management
Storage of data on secure systems operated by Big Stage Analytics and its technology partners
The program is a paid subscription service that includes hardware and ongoing software access
I am financially responsible for the wearable unit issued to my athlete. If the unit is lost, stolen, damaged beyond normal wear, or not returned when the subscription ends or the athlete leaves the program, I will pay Big Stage Analytics the then-current replacement cost. Normal practice and game use is not “damage.”
I acknowledge that data will be handled in accordance with applicable privacy laws. I release Big Stage Analytics, its partners, coaches, and staff from liability related to the normal use of the tracking devices and platform, except in cases of gross negligence or willful misconduct.
